As mentioned in Jackie's post: Loss of Origin Story/Second Combat Style Fix, you need to contact the Support Team via in-game ticket (Not a bug report) or email at support@swtor.com to get this issue resolved .

When contacting them, make sure to provide the following information:
  • Explain the that your character lost their initial Origin Story/Combat Style, so Support knows what issue you're experiencing
  • Character name
  • Server
